Medical Forms for 2016-17

Dear Parent(s)/Guardian(s) of Freshman/Transfer Incoming Student-Athletes,

Welcome to Texas A&M University-Kingsville! We are excited to have you as a member of the athletics family and are committed to providing a safe environment for your son or daughter.

Student-athletes must be medically cleared by our team physicians in order to participate in any organized strength and conditioning or practice activities. To help expedite the pre-participation physical process we ask that you fill out the contact & medical information forms listed under the "New Student Medical Forms" and submit them to Athletic Training Services by July 1st. Your cooperation in providing accurate information will allow us to register your son or daughter in our system thereby making it easier and faster to schedule his or her physical examination. The sooner you can fill out this information for your son or daughter, the sooner we can receive it and make sure everything is in place for your son/daughter's pre-participation physical process.

Health Insurance Requirement for Foreign Students

All foreign students will be required to have health insurance coverage for the entire time they are students at Texas A&M University-Kingsville. Unfortunately that information has not yet been provided by our insurance company, but we will be sure to post it as soon as it comes available.

Proper completion and submission of these forms in a timely manner is necessary in arranging for your son or daughter's physical examination when they arrive on campus. We thank you in advance for your attention to this matter.

*The information you are providing is just for Athletics and if you have given this same information before, we are requesting it because of confidentiality restrictions. Failure to return these completed forms as soon as possible will cause delays in your son/daughter's pre-participation physical exam and medical clearance to participate in athletics at Texas A&M University-Kingsville.

NEW ATHLETE MEDICAL FORMS

Please return the completed forms by July 1st, or as soon as possible. The sooner that we receive your completed medical forms, the sooner we can begin processing your information and making sure everything will be in place when your son or daughter arrives on campus.

Please Note: For the "TAMUK Authorization for Release of Information and Payment Request" form, the student-athlete must sign the form if he/she is 18 years old or older. When your son/daughter arrives on campus, it is very important that you provide them with a copy of each of their insurance cards (medical, prescription, dental and/or vision) to carry with them at all times.

RETURNING STUDENT-ATHLETE MEDICAL FORMS

Dear Parent(s)/Guardian(s) of Returning Student-Athletes,

We are excited to have you as a member of the athletics family and are committed to providing a safe environment for your son or daughter.

In order that we have your most current contact and insurance information for the new school year, please review and complete the "Returning Student-Athlete Information" documents to update your Texas A&M University-Kingsville Student-Athlete contact, insurance and health information for the upcoming year. If a form asks for a signature, we are accepting Electronic Signatures. There is no need to print it out to sign it. It is important that you keep a copy of all forms for your records.

NOTE: This year we are again requiring Prescription Insurance Information, so that form will need to be scanned or sent in this year as well as a FRONT and BACK Enlarged to 130% copy of your prescription card. If your contact or insurance information changes as the year goes forward, please submit the form with your changed insurance cards as soon as the changes take effect.
